This service provides nursing care and support in the community for children with complex medical conditions. The care is provided by trained Health Care Assistants under the supervision of a qualified Children’s community nurse. Access to this service is determined by a nursing assessment and gaining information from all the medical staff and key workers involved in their care. This information is then examined against the Department of Health’s Complex healthcare decision making tool, ensuring that access to the service is equitable. The carers can provide support for the child in the home, at school and in respite facilities. This service can provide care 24 hours a day 7 days a week.
