The looked after children health team is a small team of experienced nurses who co-ordinate healthcare for children in care. This includes both co-ordination of statutory health assessments carried out by GP’s paediatricians, and public health nurses, and also some direct health assessments for children / young people who are harder to reach.
The team relies on information provided by Devon County Council Children and Families services on children/young people received into care, moving placements within care or discharged from care.
There is a statutory requirement that all children in care receive regular health assessment and health care planning (for children under the age of 5 years, this is 6–monthy, and on an annual basis there after).
