All Primary Care Trusts aim to provide the best possible services for children and young people who have life limiting or life threatening conditions.

NHS Devon, NHS Plymouth and Torbay  Care Trust have therefore collectively signed up to the princliples outlined in the Children and Young Person's Palliative Care Charter  and have developed an Advanced Care Planning Tool as well as specific children and young people's pages on the Adult palliative care register.
